bought the second edition a while back in med school and rocked my EM rotation using this book. i got a free copy of this as a gift and this latest edition continues to be solid.

if there was ever an equivalent to surgical recall for emergency medicine this is it.

the first few chapters are on chief complaint based workup for the most common chief complaints. that alone is worth its weight in gold. emergency medicine is all about starting with a chief complaint and working towards a diagnosis or at least ruling out the critical diagnoses you cannot miss. this style of thinking is counter to the traditional teaching of medicine where you learn a disease first and the symptoms and physical exam findings and diagnostic lab/x-ray findings consistent with it. this is real medicine though. this is how you should be thinking about patients.

I picked this up for a 1 month MS4 elective in EM when I was searching for a useful handbook (something like the Pocket Medicine "Green [or red, or purple] Book") The EM "Pocket" series handbook was poorly reviewed by classmates, so I picked up "Secrets" based on Amazon reviews.

So far: it's not a useful handbook of algorithms, differentials, lab interpretation etc like the Green Book, but it is a very good text for the clinical rotation. Most clinical teaching on the EM rotation so far has focused on the "philosophy" of medical decision making and management in the ED, and this book covers those topics effectively. It covers crucial information about all the common symptoms and disorders you see in the ED, and the "pimp question" format makes it easy to remember. I've been asked similar questions as those in the book, and if I've already read the relevant material, I feel like a rock star.

Overall, this text covers a lot of useful information, is organized well, is easy to read, and fits (heavily) in a white-coat pocket for glancing through a chapter before writing a note. On the downside, it's denser than I had hoped for a 1 month rotation, and it's not useful for quickly looking up guidelines as you go like a good handbook would be. I can't comment on its usefulness for the exam yet, but as exams can vary widely from place to place, you'll have to make your own judgment about what sort of test you'll be taking. If you're looking for a good, comprehensive-enough, text for a required or elective ED rotation as a medical student, this book is a winner. If you are looking for a handbook, try one of the various websites that provide EM evidence-based algorithms.
