I always carry this book with me, it gets straight to the facts and helps answer any questions that I may have.
I highly recommend this book to any one who needs help documenting, or who would like to improve their documentation skills.

I ordered this book because I always have problems with charting on my patients - have I included too much, not enough, did I forget something?? I bought this to help with assuring that everything is charted and charted correctly on all of my patients! I was thrilled that this was available in the Kindle version but SO disappointed after I ordered it!!! The table of contents list "DOCUMENTATION (in alphabetical order)" which is great if you are looking up something beginnin with an "A" or "B" but if you are searching for let's say "Tuberculosis" you have to use the bottom scroll (location ??) to scroll towards the end of the book and THEN go page by page by page until you finally locate the "T" section and THEN keep going page by page by page until you have finally reach "Tuberculosis"! There is no real "Table of Contents" to speak of AT ALL! If I had known it was set up this way I would have bought the print version (which I will probably end up doing anyway). VERY DISAPPOINTED!!
